Conditions when a Filipino Citizen can apply for a Travel Document:

  1. In case of a lost Philippine passport
  2. In case of absence of Philippine passport (for infants born in Korea)
  3. In case of expired Philippine passport
  4. In case of mutilated Philippine passport
NOTE: A Travel Document is issued in lieu of a passport under the above circumstances and valid for one-way direct travel to the Philippines.

TRAVEL DOCUMENT IN LIEU OF LOST/MUTILATED PASSPORT:
Requirements:
 
  1. 4 pcs. passport-size pictures of the applicant
  2. Duly accomplished Affidavit of Lost Passport form
  3. Duly accomplished Travel Document application form
  4. Copy of lost Philippine passport, if available
  5. If there is no available copy of the lost Philippine passport, submit any of the following: NSO/PSA issued Birth Certificate, Alien Registration Card, NBI Clearance, or at least 2 government issued documents/IDs indicating the applicant’s full name, date and place of birth, gender
TRAVEL DOCUMENT FOR INFANTS WITH NO PASSPORT:
Requirements:
 
  1. 4 pcs. passport-size pictures of the applicant
  2. Duly accomplished Travel Document application form
  3. 1 set of Report of Birth documents notarized by the Philippine Embassy-Seoul
NOTE: If the child will travel with neither of the parents, the parents should execute an ‘Affidavit of Parental Consent’ to be signed personally by both parents of the child and the travelling companion. In addition to the affidavit, a photocopy of the passport of the child’s travelling companion is also required, as well as the personal appearance of both parents and travelling companion.

TRAVEL DOCUMENT IN LIEU OF EXPIRED PHILIPPINE PASSPORT:
Requirements:
 
  1. 4 pcs. passport-size pictures of the applicant
  2. Duly accomplished Travel Document application form
  3. Original and 1 photocopy of the expired Philippine passport
