GENERAL REQUIREMENTS

  • Personal Appearance;
  • Confirmed appointment;
  • Police report with English translation
  • Submit Affidavit of Loss (with detailed explanation on when, where and how passport got lost);
  • Photocopy of first page of lost passport (if available)
  • Duly accomplished application form No need to submit a passport size photo;
  • Fee for a lost Machine Readable Passport is US$ 90.00 or W 120,780. For lost ePassport, fee is US$ 150.00 or W 201,300;
  • Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) and duly authenticated by the DFA or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by the DFA. Transcribed Birth Certificate from the LCR is required when entries in NSO Birth Certificate are blurred or unreadable.? If born abroad, applicant may submit Report of Birth duly authenticated by NSO;
  • Two Government-issued picture IDs to prove identity
  1. Digitized SSS ID
  2. Driver's License
  3. GSIS E-card
  4. PRC ID
  5. IBP ID
  6. OWWA ID
  7. Digitized BIR ID
  8. Senior Citizen's ID
  9. Voter’s ID
  10. NBI Clearance
  11. Original previous passports
  12. CFO Certificate (with picture printed on certificate)

* Please take note that there will be a 15-day clearing period prior to the processing of application for replacement of a lost valid passport;

ADDITIONAL REQUIREMENTS

  • For Married Women who want to use surname of husband:
  1. Marriage Contract (MC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or original and photocopy of the Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. Transcribed Marriage Contract from the LCR is required when entries in NSO Marriage Contract are blurred or unreadable; or Report of Marriage duly authenticated by DFA or Philippine Embassy/Consulate if married abroad
  • For spouse of foreign national:
  1. Original and photocopy of Commission of Filipino Overseas (CFO) Guidance and Counseling Certificate of Attendance (required for first time applicants or renewal of passport to be used for the first time the surname of the husband)
  • Applicants who availed of Dual Citizenship or Election of Philippine Citizenship under RA 9225:
  1. Identification Certificate
  2. Oath of Allegiance
  3. Order of Approval
  4. Report of Birth (for those born abroad)
