ACKNOWLEDGEMENTS AND NOTARIALS (only for Filipino nationals):

Requirements:

  1. Personal Appearance;
  2. Duly accomplished Acknowledgement and Notarial forms (may be downloaded from http://philembassy-seoul.com/forms.asp)
  3. Original and photocopy of valid Philippine Passport; for dual citizens, original and photocopy of Identification Certificate issued by the Philippine Embassy or Philippine Bureau of Immigration
  4. Payment of KRW 33,550 or USD 25 per document

NOTE: Documents are released after three (3) working days. For one-day processing, an additional fee of KRW 13,420 per document will be collected.


AUTHENTICATION OF DOCUMENTS ISSUED IN KOREA:

Requirements:

  1. For documents in Korean, translated copy (in English) of the document and duly notarized by a Korean Notary Public and authenticated by the Ministry of Foreign Affairs in Korea
  2. Payment of KRW 33,550 or USD 25 per document;

NOTE: The Embassy of the Philippines accepts authentication applications in the morning from 9:00 AM to 12:00 NN ONLY. Releasing of documents starts at 1:00 PM to 3:00 PM. Documents are released after three (3) working days. For one-day processing, an additional fee of KRW 13,420 per document will be collected.


AUTHENTICATION OF DOCUMENTS ISSUED IN THE PHILIPPINES TO BE USED IN KOREA:

Documents issued in the Philippines for use in Korea must go through the following procedures in consecutive order:

  1. The document must be authenticated by the issuing agency in the Philippines
  2. The document must be authenticated by the Department of Foreign Affairs (DFA) in Manila
  3. After DFA authentication, the document may either be authenticated in Manila by the Korean Embassy or in Korea by the Philippine Embassy in Seoul.

  • National Statistics Office (NSO) -issued documents on security paper (e.g. birth certificate, marriage certificate, certificate of no marriage)
  • Affidavits with Regional Trial Court Certification
  • Notarized Documents with Regional Trial Court Certification
  • Medical Certifications authenticated by the Department of Health
  • Professional Regulation Commission documents
  • Land Transportation Office (LTO) License Certification issued by LTO main office only
  • Foreign documents authenticated by a Philippine Embassy or Philippine Consulate-General
  • Foreign documents authenticated by a foreign Embassy or foreign Consulate-General in the Philippines
  • School Records (transcript of records, diploma, Form 137, etc) with a Certificate of Authentication and Verification (CAV) from the Commission on Higher Education, Technical Education and Skills Development Authority or Department of Education
  • School Records with a Certificate of Authentication and Verification (CAV) issued by a State College or State University

NOTE: Documents may be submitted for authentication by an authorized representative in the Philippines. The authorized representative must have a valid identification and must present the following: 1. Signed authorization letter or Special Power of Attorney from the applicant; 2.Photocopy of applicant’s identification bearing the applicant’s signature, and; 3.Photocopy of the authorized representative’s identification.


AFFIDAVIT OF PARENTAL CONSENT FOR TRAVEL OF MINOR (for minors traveling from Korea to the Philippines):

Requirements:

  1. Personal appearance of any of the Filipino parent (for married parents) or both parents (if not married), and the traveling companion (for interview by the Social Welfare Attach?)
  2. Duly accomplished Affidavit of Parental Consent form
  3. Photocopy of valid passport of the parent(s) and the traveling companion
  4. Photocopy of the birth certificate of the child
  5. Photocopy of the passport of the child (if available);
  6. Payment of KRW 33,550 or USD 25

NOTE: Documents are released after three (3) working days. For one-day processing, an additional fee of KRW 13,420 per document will be collected.


PETS ENTERING THE PHILIPPINES:

Those who wish to bring their pets from South Korea to the Philippines must first get an IMPORT PERMIT from the Bureau of Animal Industry.

Notary services are available for Filipino nationals only. A photocopy of the first and last pages of the passport is required alongside the document to be notarized. Take note that only affidavits and special powers of attorney can be notarized. For details, please call (+82) (2) 796-7387, ext. 106 or 108. 

For documents in Korea that need to be authenticated, a translated copy in English notarized by a Korean lawyer and authenticated by Korea's MOFAT is necessary plus one photocopy of each document. The officer from MOFAT's signature, which attests to the contents of the document, is what the Embassy will authenticate. For details, please call (+82) (2) 796-7387.

  2. HEALTH CERTIFICATE issued by a duly licensed veterinarian dated not more than 30 days before arrival in the Philippines. It should certify that the animal is free from, and has not been recently exposed to, any dangerous or communicable disease, and that it has been given anti-rabies and other required inoculations.
  3. The Health Certificate should be notarized by a Korean Notary Public and authenticated by the Ministry of Foreign Affairs of Korea before you present to the Philippine Embassy for authentication. Fee is KRW 33,550 or USD 25 to be released after three (3) working days. You may request for earlier release for an additional payment of KRW 13,420 to be released on the next working day

TRAVEL TAX EXEMPTION CERTIFICATE:

NOTE: Registration of Embassy ID is FREE and are released after three (3) working days.


CERTIFICATION OF PHILIPPINE DRIVER’S LICENSE:

FOR FILIPINO NATIONALS

Requirements:

  1. Personal Appearance
  2. DFA-Authenticated Certification from LTO
  3. Original and photocopy of valid Philippine Driver’s License
  4. Original and photocopy of valid Alien Registration Card (ARC) from Korea
  5. SOFA Visa (for US Military Spouse only)
  6. Original and photocopy of valid Philippine Passport
  7. Copy of passport page showing Philippine Immigration stamps (showing entry into, and exit from, the Philippines during dates of renewal of license)
  8. Payment of KRW 33,550 or USD 25

NOTE: Expired driver’s license can only be renewed at any LTO branches in the Philippines NOT at the Philippine Embassy. Certification is released after three (3) working days. For one-day processing, an additional fee of KRW 13,420 per document will be collected.

FOR NON-FILIPINO NATIONALS

Requirements:

  1. Personal Appearance
  2. DFA-Authenticated Certification from LTO
  3. Original and photocopy of valid Philippine Driver’s License
  4. Original and photocopy of valid Passport
  5. Copy of passport page showing Philippine Immigration stamps (showing entry into, and exit from, the Philippines during dates of renewal of license)
  6. Payment of KRW 33,550 or USD 25

NBI CARD FINGERPRINTING:

The NBI form is not a consular document. However, the Philippine Embassy assists Filipinos who are applying for NBI clearance in terms of fingerprinting on the card form. Upon completion of the requirements below, the applicant must send the form, together with an authorization letter, to his/her authorized representative in the Philippines, who will process the clearance with the National Bureau of Investigation (NBI) on your behalf.
